NYC-CDRN Lifestyle and Weight Research Summary

Investigator: Dr. Ana Emiliano (Rockefeller), Dr. William Pagano (Lutheran FHC), Dr. Jonathan Tobin (Clinical Directors Network)

Goal: We want to learn more about how different factors and experiences come together in people managing weight issues.

About the survey: The survey is made up of 25 questions about managing weight issues and/or undergoing bariatric surgery. It will take around 10 minutes to complete the survey. It also contains questions about what you think about research studies in general. We are asking people who may have been eligible for bariatric surgery and who have been seen for a visit at one of the New York City Clinical Data Research Network participating hospitals or clinics to participate in the survey. Learn more about the NYC-CDRN. After we have collected your survey answers, we would like to link your survey answers to information in your medical records. The NYC-CDRN has developed a secure process to combine medical records across multiple institutions and other data sources like health plans in a way that protects your privacy.

We hope that what we learn will help us figure out ways to improve the health of people managing weight issues.
